A company is looking for an RN Case Manager for Transitions of Care.
Key Responsibilities:
Engaging with newly discharged members telephonically to assess needs and develop care plans
Coordinating post-treatment care, referrals, and collaborating with healthcare providers
Educating members on chronic conditions and providing active care management
Required Qualifications:
Unrestricted compact RN license in the United States
Minimum of 4 years of RN experience in case management or home health
Desired experience in health insurance, telephonic care, or acute care
